I am new to puppet and have been tasked to set up a module that will install and manage php. I have located a modules on puppet forge, but it does not have an init.pp file. Do I need to create one or does puppet apply use another file to perform the install?

Could you point us to the forge module in question? Not having an init.pp is unusual.

I just recently started evaluating modules for this exact thing and the mayflower php module is pretty good

https://forge.puppetlabs.com/mayflower/php

It didn't cover my use case (compiling php with some specific options) but if you can use php from your local package management repo it seems allright.
