What documents or questionnaires do you use for requirements management?

I have been recently tasked with putting together a project plan to puppetize an existing piece of legacy software. We already use puppet throughout our enterprise at different levels of integration and adoption, and this is yet another piece of technology yet to go under the umbrella.

This is a piece of internally developed code, a bunch of java, a bunch of tomcat, a bunch of wacky jar files, system service requirements, etc... I you receive something like this, I'm also quite interested in this kind of document... we are also just trying to put puppet into our organisation.

Microsoft has Team Foundation Server with project management tools, if you are already using Microsoft tools.

With Github I use the Wiki to document concepts about the project and the file to document module usage. With Jira I track work broken out into stories (things you can do in a week's time) and their acceptance criteria (things we can test to say we are done). The prject plan is managed through the built in reports and grouping stories into epic (which are the miletones for your project).
