Error: Could not find certificate request for pe-internal-dashboard

I am trying to install puppet enterprise 3.0.1 on RHEL5 with Master, Console and DB on same node but my installation process terminates with following errors:

It looks like the dashboard didn't start up, so a request isn't being created. I'd look in the http logs to see why it didn't start.

The http server did start but its throwing HTTP 500. Dashboard logs show this error: http://pastebin.com/dNrgXwjq and apache logs show: http://pastebin.com/XVThjtMz
